Applications must include the following:

Teacher Recommendation Form from at least two (2) current academic teachers (Math, Science, English, or Social Studies) submitted in a signed sealed envelope

Applicant Essay (must follow format as described in application)

Mass-Media, Print and Graphic Image Release
